ASCRS MIDDLE SEGMENT SURGERY DAY

Join leading anterior segment surgeons for a full day of education on the cases where the anatomy stops cooperating, with practical strategies you can take back to your OR.

When the Limbus Isn't Enough

When lenses dislocate, capsules fail, and the anatomy no longer cooperates, today's anterior segment surgeon needs more than a limbal approach. This inaugural program explores the expanding role of middle segment surgery, from IOL exchange and intrascleral haptic and suture fixation to dead bag syndrome, zonulopathy, iris reconstruction, and combined cornea and glaucoma procedures.

Through expert presentations, surgical videos, and interactive Q&A, you'll gain practical strategies for managing complex cases through the pars plana. In the afternoon, don't miss a live surgery session broadcast from Codet Vision Institute in Tijuana, Mexico, featuring Shin Yamane, MD, in the OR.

Opening Session: Framing the Middle Segment

Explore the middle segment approach and why it matters for anterior segment surgeons. Dr. Shin Yamane will deliver the keynote on the evolution of his intrascleral haptic fixation technique, a procedure that has transformed secondary IOL surgery worldwide

Session I: Lens Exchange, Fixation, and Capsular Failure

This session covers the core techniques every middle segment surgeon needs, from IOL exchange with an open posterior capsule to primary and secondary intrascleral haptic fixation. Faculty will address recognizing and managing dead bag syndrome, compare suture fixation approaches, and explain when and how to use Gore-Tex suture, with practical pearls on centration, tilt, and rescue maneuvers.

Session II: Complex Reconstruction and Combined Procedures

Building on the fundamentals, this session tackles the most challenging cases: zonulopathy with vitreous prolapse, dropped crystalline lenses and posteriorly dislocated IOLs, and combined iris reconstruction with IOL fixation. Experts will also discuss staging versus combining secondary IOL surgery with corneal transplantation, integrating glaucoma procedures, and pars plana approaches to aqueous misdirection.
